Guidelines for submitting Publishing Agreement Form and Permission Request Form
1. Download the following forms
a. Publishing Agreement Form
b. Permission Request Form
c. SUSCOM-2021 Registration Form
2. Publishing Agreement form along with payment proof must be submitted through SUSCOM-2021 Registration Form
3. Permission Request Form is applicable to those who have taken excerpts from copyrighted works (including websites) such as illustrations, tables, animations, or text quotations and included in the manuscript. Please obtain permission from the copyright holder (usually the original publisher) for both the print and online format. If you have not included any such material then there is no need to submit the permission request form

Guidelines for submitting Publishing Agreement Form and Permission Request Form
1. Corresponding author should enter the names of all the authors including him/herself at the top of the form in the following format:
First Author Name, Affiliation, Email ID
Second Author Name, Affiliation, Email ID
...
Please note the name and order of the authors should be the same as mentioned in paper
2. Then enter the Name of the Corresponding Author (Name of only one author)
3. Enter the title of the paper at the appropriate place
4. Print the form, sign it and upload in .PDF format through SUSCOM-2021 Registration Form
